Or better yet: He decides to take his life with a suicide pill. Once he's taken it, and starts feeling the effects, he decides to open the door to have one last look at the world and die in the now wasteland that was once his home.

When he realizes that the world is still alive and well, it's already too late to stop the effects of the suicide pill. He dies, his physical agony drowned out only by the realization that he wasted all those years, dragged his fellows with him and fought and killed them for nothing.

His death is never noticed in the media. The authorities fail to identify him and his death is written off as any another suicide. The bunker with his dead fellows is never found.

There's no trace of this fatal tragedy. It may just as well never have happened.

It was actually a thing from the beginning in Vault 11, with the first Overseer being the only one who initially knew about the sacrifice. When the other dwellers found out ABOUT THE SACRIFICE, it just became tradition to have the Overseer and the sacrifice be one and the same.

I'm pretty sure bunkers capable of maintaining an independent supply of water, power and food for decades don't come cheap. I mean, at least you'd have to buy 50 years of groceries at once. And you'd probably have to used specialized stuff with better sealing and reduced oxygen content.

A man goes in with 10 survivors, his close friends and daughter. The years go by and it’s vault life, shit sucks, there’s nothing much to do, you hate it. On the yearly supply check you notice something bad, it’s running out. You realize it can’t feed all of you, it’s a good enough supply for a few people, but not 10. You do what you have to, it’s you and your daughter that matter. You pretend to hold a meeting in a new spot in the vault for a special occasion. In the room, your friends are wondering why it smells bad, you lock the door and turn on the incinerator. It’s a solid heavy door, you can’t hear the screams, but you cry anyway. You go to your daughters room, in there you found the note you wrote to her explicitly telling her not to go to the meeting. She must’ve forgot to read it. You cry, you’re alone, you’re angry at yourself, you have to take your own life, you decide to leave the shelter, just to see what happened. The war never happened. Your old town is the same. You cry.

I feel like this was a movie, with Christopher Walken. During the cold war when the threat of nuclear war was pretty high, there's an emergency broadcast and he takes his family into a fallout shelter he had built thinking the bombs were about to fall. They live there for like a couple decades then send the son out to get supplies because they're low on food. Can't remember the name but it was a neat movie.

Governor Ige said human error during one of the thrice-daily shift changes at the state's Emergency Management Agency (EMA) was to blame for the false alert.

Hawaii isn't that big, you'd probably be able to tell when a missile hit from the shockwave or the tremors.

In September there were reports of North Korea being shaken by two tremors after a nuclear test and it's pretty similar in size (like 300km across NK and 300km between the big island and Honolulu)

Well, in the film the the issue was that a plane crashed on the house of the people in the bunker. Which molt their surface sensors.

You're also not actually capable to notice these tremors yourself. For that you really need a seismograph and ways to make sure your toddler in the next room stomping isn't mistaken for an earthquake. Not easy to do.

Really, the nuclear tests lead to tremors of 5.something on the Richter-Scale. If you could notice that in 300km distance, you could notice earthquakes above 8 on every place on earth. But that's not the case.

Edit: Also those were underground tests. Airbursts do obviously not even cause a fraction of the seismic waves. So to notice a nuclear blast it would make sense to react to the radiation (they're quite bright).
